Implementing the Population Health Approach
Implementing a Population Health Approach:
-
is a cyclical process - and there's not necessarily a linear step by step
way to apply the method to project work. You can start anywhere on the wheel,
and go over the same territory in cycles. The roles of the partners evolve
over time.
