CHRONIC DISEASE

Integrated Approaches to Chronic Disease

Integrated approaches to chronic disease will increase action in health promotion, illness prevention and care. Proposals developed under this priority will identify strategic directions on chronic disease using a population health approach. This could include:

  • improved use of or access to services and resources;
  • increased action to combine or balance efforts in preventive health practice and health promotion; and
  • increased action to cooperate and respond to population health and needs among high-risk groups in the prevention and control of chronic disease.

Best Practices

Knowledge of how and why chronic disease prevention and control programs work, and what difference they make, needs to be brought together. Projects under this priority will build, transfer or communicate knowledge in this area across sectors or disciplines. Proposals developed in response to this priority will develop, analyze, collect or disseminate best practices.

Demonstration Projects

Demonstration projects test and communicate the results of interventions for chronic disease prevention and control. Projects under this priority must:

  • produce tangible results;
  • disseminate information on interventions;
  • show clear potential for contributing to pan-Canadian policy on chronic disease prevention and control; and
  • provide opportunities to build skills and training tools that could be used by other areas in Canada.

Projects may develop and support activities at local, regional and provincial levels, but must identify mechanisms for sharing experiences among these jurisdictions.

Chronic Disease and Health Promotion Networks, Coalitions and Alliances

Networks, coalitions and alliances make population health information widely available and usable. They can play a role in sharing information and evidence with partners and the public. Projects addressing this priority will:

  • increase the focus on chronic disease prevention in decision making;
  • increase capacity to act on chronic disease; and
  • advance population health analysis in coalitions or networks concerned with chronic disease.

Projects could also develop strategic directions and policy analysis on the determinants of health and chronic disease.

BEST PRACTICES

In response to the PHFN's program evaluation and stakeholder consultation, this solicitation supports the continuation of successfully completed PHFN-funded projects that fit a current priority through the theme of Best Practices.

The PHFN defines Best Practices as having four elements:

  • innovative - they have developed new and creative solutions;
  • make a difference - they have demonstrated a positive impact;
  • sustainable - they continue even after the original funding has ended; and
  • potential for replication - they can serve as models for generating policies and initiatives in various locations.

Proposals for best practices projects should focus on one of the following options:

  1. strengthening the evaluation component of a completed project that has the potential to be identified as a best practice; or
  2. supporting the replication of a completed project that has already undergone a rigorous evaluation and could be considered a best practice.
