Complete the order form below to order the title(s) you wish to borrow. A pull-down menu is available in each "Title" field, indicating all available titles. For producer and/or format information, refer to the following list. Please note that films will only be shipped within Canada and the United States. Also note that some titles are in very limited supply. All orders are first come, first served.

1. Please use this form to expedite your film loan request.
2. If you do not indicate a preferred date, we will ship the film as soon as it is available.
3. Where applicable, please indicate which video cassette format is required.
