Diabetes Rural Community-Based Program in Petitcodiac and Surrounding Area is a 12-month project of New Brunswick’s South-East Regional Health Authority. The purpose of the project is to increase the capacity of public health and community stakeholders to apply best practices in type 2 diabetes prevention in rural communities. The agency will conduct a literature review to identify best practices. Focus groups, key informant interviews and surveys will be used to identify the needs and interests of stakeholders in the region’s specific rural communities. With this information, the agency will develop and implement a plan for a diabetes self-management and risk reduction program that responds to the need of their rural communities.

Un échange intergénérationnel is a 12-month project of the Association acadienne et francophone des aînées et aînés du Nouveau-Brunswick. The purpose of the project is to increase awareness of type 2 diabetes prevention among seniors and youth in New Brunswick’s francophone communities. Under the guidance of an advisory committee composed of youth and seniors, the group will develop educational material and organize a series of school health days. Youth and seniors together will then lead health day activities in francophone high schools around the province, focussing on the impacts and prevention of type 2 diabetes.

Information en main, santé pour demain is a 12-month project of la Société éducative de l'Île-du-Prince-Édouard. The purpose of the project is to increase awareness of type 2 diabetes prevention and management among Prince Edward Island’s francophone and Acadian population. The group will inventory the province’s existing programs and services related to type 2 diabetes, identify those programs and services available in French and work with partners to adapt others to better respond to the needs of the Acadian and francophone population. The group will then produce a guide to programs and service providers and conduct a public awareness campaign to make this information available to the community.

La lutte contre le diabète: des solutions innovatrices en prévention primaire pour la population rurale (english translation here) is a 12-month project of la Régie régionale de la santé Beauséjour. The purpose of the project is to increase early detection of type 2 diabetes among rural francophone, Acadian and Aboriginal communities in Kent County, New Brunswick. The agency will develop a public awareness campaign and information kit on diabetes prevention and early detection. Opportunities for early detection and awareness will be provided through various community meeting and work places. Those found to be at high risk for diabetes will be referred to appropriate medical services.

Taking Hold of Our Health - African Nova Scotian Youth Diabetes Prevention Project is a 12-month project of the Health Association of African Canadians. The purpose of the project is to raise awareness of type 2 diabetes prevention among African Nova Scotian youth, families and communities. Building and strengthening partnerships for promoting health in the Black community will also be a priority. The group will work with Black youth to develop a culturally-relevant diabetes awareness and prevention education program using a train-the-trainer model. A public event will be organized for youth participants to share their learning with African Nova-Scotian communities. At the close of the project, the culturally-relevant type 2 diabetes education program will be available for future use with African Nova Scotian youth.
